Objectives

The primary objective of this tender is to procure 114 units of Snellen Chart for paediatric eye assessments, ensuring the hospital has adequate equipment for eye testing in children.

Scope

The scope includes the supply of 114 units of paediatric Snellen Charts, compliant with specified standards and specifications, to Prince Mshiyeni Memorial Hospital in KwaZulu-Natal. The procurement covers the entire process from quotation submission, evaluation, to delivery at the hospital's designated location.

Technical Requirements

• Design of optotypes on a 5x5 grid with line thickness of one unit and letter dimensions of five units for standard charts.
• Inclusion of children's test symbols such as Allen pictures, with thinner lines as appropriate.
• Samples may be requested by the institution for verification.
• Conformance to specified standards and specifications outlined in Annexure A.
• Delivery in packaging suitable for safe transit, with proper marking and documentation.
• Warranty of 12 months for the supplied goods, with provisions for repair or replacement of defective items.

Skills and Skills Requirements

• Understanding of medical device specifications and standards, particularly for ophthalmic testing equipment.
• Ability to prepare compliant samples and documentation as per technical specifications.
• Knowledge of procurement procedures, including submission of official forms and adherence to evaluation criteria.
