Objectives

The primary objective of this tender is to procure numbered stickers (No 2, No 3) and A4 clear plastic sleeves that meet specified standards and requirements for use at Ladysmith Regional Hospital, under the Department of Health in KwaZulu-Natal. The procurement aims to ensure the supply of quality items that conform to technical specifications, supporting hospital administrative and operational needs.

Scope

The scope encompasses the supply of:

• 20,000 rolls of Number 2 stickers
• 20,000 rolls of Number 3 stickers

and

• Plastic sleeves in A4 size, with a quantity of 100 sleeves per packet, as detailed in the specifications.

The items are to be delivered once, with the procurement process including technical evaluation, price assessment, and compliance with departmental policies.

Technical Requirements

The items must meet the following specifications:

• Numbered stickers (No 2 and No 3):
• Size: Oval shape, approximately 23 mm long
• Design: Blue numbers on white paper
• Quantity: 20,000 rolls each
• A4 Plastic Sleeves (Clear):
• Pack size: 100 sleeves per packet
• Features: NFS 2 & 4-hole punching, suitable for filing and storage
• Quantity: 6 packets (or as specified)

Suppliers must ensure items conform to SABS or SANAS standards and provide samples if required. All items should be new, unused, and of the latest models, with warranties as specified.

Skills Requirements

The tenderer must demonstrate:

• Technical capability to produce items that meet detailed specifications and quality standards.
• Experience in supplying hospital or similar institutional supplies.
• Knowledge of relevant standards (SABS/SANS) and compliance procedures.
• Ability to provide technical support, documentation, and timely delivery.
• Disclose any interest or relationship with the Department of Health or associated personnel, ensuring transparency and compliance with procurement policies.
