Objectives

The primary objective of this tender is to procure and supply sets of patient referral letters in accordance with the specified standards and specifications, ensuring quality, compliance, and timely delivery to Hlabisa Hospital.

Scope

The scope of the tender includes:

• Supply and delivery of 200 packets of patient referral letters, each consisting of 4 leaves (triplicate and quadruplicate forms), sized 297mm x 210mm, printed in black ink on white 60gsm paper.
• Provision of related documentation, including specifications, sizes, and evaluation criteria.
• Adherence to technical standards and quality requirements as specified in the tender documents.
• Delivery to Hlabisa Hospital main gate tender box at 60 Saunders Street, Hlabisa, within the contract period (once-off).

Technical Requirements

The technical requirements for the patient referral letter sets are as follows:

• Each set must contain 4 leaves: original (white NCR CB perforated), duplicate (white NCR CFB perforated), triplicate (blue NCR CFB), and quadruplicate (yellow NCR paper).
• Gummed across the top, perforated (mill-tinted), with size 297mm x 210mm.
• Printed in black ink on white 60gsm paper, side only.
• Each packet must contain 50 sets of referral letters and one guideline form.
• All items must be boxed for delivery.
• Compliance with the attached specifications, sizes, and evaluation criteria as per Annexure A.
• Attachment of BBBEE certificate or sworn affidavit is mandatory.

Skills Requirements

The tenderer must demonstrate the following skills and capabilities:

• Ability to produce high-quality, standard-compliant NCR forms with precise perforation and printing specifications.
• Experience in manufacturing or printing multi-leaf forms with accurate color coding and size specifications.
• Capacity to package and deliver the products securely and promptly to the specified location.
• Understanding of procurement procedures, quality standards, and compliance documentation.
• Ability to provide technical support and documentation as required, including specifications and evaluation criteria.
