Scope

This is a once-off quotation for the supply of 500 packets of brown storage covers specifically intended for patient file documentation. Deliveries must be made directly to Northdale Hospital in Pietermaritzburg. The contract requires firm pricing valid for 90 days, with all transportation and handling costs included in the quoted amount. Expired, second-hand, or non-compliant products are strictly excluded. The evaluation follows a five-stage process covering administrative compliance, capacity, specification adherence, pricing/preference scoring, and physical sample verification.

Technical Requirements

• Product must be brown in color and suitable for patient documentation storage.
• Packaging must consist of 100 units per packet, with 8 packets per box.
• All supplied items must have a minimum validity period of six months from the date of delivery.
• Bidders must use the official quotation forms without retyping; original wet signatures are mandatory.
• Top-scoring responsive bidders will be required to submit physical samples for inspection prior to contract award.
• Full compliance with tax clearance, Central Supplier Database (CSD) registration, and mandatory returnable documents is required.

Cover-Quoting: A prohibited anti-competitive practice where multiple companies submit bids for the same contract to artificially influence outcomes. Detection leads to immediate disqualification and potential blacklisting.

Performance Security & Delay Penalties: Awarded suppliers may be required to provide a bank guarantee or certified cheque. Late deliveries incur financial penalties calculated daily using the current prime interest rate until fulfillment.

Sample Submission Protocol: An objective evaluation stage where the highest-scoring bidders must physically submit product samples. If samples fail inspection, the procurement process restarts. Samples are requested via email after initial scoring.
