Objectives

The primary objective of this quotation is to procure a reliable, compliant supplier for the one-off provision of disposable medical speculum instruments to St Andrews Hospital, operating under the KwaZulu-Natal Department of Health.

Scope

The contract covers the supply and delivery of 300 units of disposable Cusco speculums, allocated across three size categories:

• Small: 100 units
• Medium: 100 units
• Large: 100 units

Delivery must occur at St Andrews Hospital. The agreement is structured as a single-delivery contract with a quotation validity period of 90 days from the closing date.

Technical Requirements

• Supplied goods must be brand new, unused, and conform to applicable South African National Standards (SANS/SABS).
• All products must retain a minimum shelf life of six months upon delivery; expired stock will be rejected.
• Bidders must explicitly confirm compliance with the attached technical specification. Failure to do so results in immediate disqualification.
• Samples may be requested for quality verification. Non-submission when requested leads to rejection.
• Quoted prices must be firm, all-inclusive, and cover all logistics and delivery costs to the specified hospital address.
• A mandatory warranty applies: twelve (12) months from delivery/acceptance or eighteen (18) months from shipment, whichever expires first.

Skills & Business Requirements

• Active registration and successful verification on the Central Suppliers Database (CSD).
• Valid tax compliance status, verifiable via a SARS PIN or Tax Clearance Certificate.
• Current Broad-Based Black Economic Empowerment (B-BBEE) status level certificate or a sworn affidavit (for EMEs/QSEs).
• Official company registration documents (CK series).
• Completion of mandatory statutory declaration forms, including the Bidder’s Disclosure of Interest (SBD 4) and Preference Points Claim Form (SBD 6.1).
• Financial capacity to furnish performance security (bank guarantee or certified cheque) within thirty days of contract award.
• Prohibition on subcontracting without prior written consent from the Department.

• 80/20 Preference Point System: Evaluation scoring allocates 80 points to price competitiveness and 20 points to transformation goals. Full points for the specific goal are granted exclusively to enterprises with ≥51% Black ownership, substantiated by CIPC records or a sworn affidavit.
• Returnable Documents: Prescribed government quotation forms that must be submitted exactly as printed. Retyping, redrafting, or altering these forms invalidates the submission.
• Performance Security: A financial guarantee required post-award to mitigate supplier default risk. It ensures contractual obligations are fulfilled.
• Delay Penalties: Late deliveries trigger daily financial deductions calculated using the current prime interest rate applied to the value of undelivered goods.
• Restriction Penalty: Engaging in fraudulent, corrupt, or collusive practices may result in a prohibition from contracting with any public sector entity for up to ten years.
