Objectives

The primary objective is to procure high-quality, sterile disposable microsurgical blades for ophthalmic procedures at Northdale Hospital. Scope

This is a one-off supply contract for 20 boxes of 15-degree blades to be delivered to Northdale Hospital in KwaZulu-Natal. The quotation validity period is 90 days, and all prices must remain firm. The scope includes delivery to the prescribed destination, provision of samples upon request, and strict adherence to regulatory, quality, and administrative requirements. Expired, second-hand, or non-compliant products will be rejected.

Technical Requirements

• Product Specifications: Sterile, single-use microsurgical blades made from surgical-grade stainless steel, featuring plastic/polycarbonate or integrated handles.
• Packaging & Labelling: Supplied in peel-open sterile pouches, clearly labelled for theatre identification, and packed in boxes of 6 or 10 units.
• Quality & Shelf Life: Must meet the highest quality standards, be brand new, and retain a minimum of six months’ shelf life upon delivery.
• Regulatory Compliance: Suppliers must hold a valid licence issued by the South African Health Products Regulatory Authority (SAHPRA) permitting the manufacture, wholesale, or distribution of medical devices.
• Documentation: Completion of all official quotation forms, SBD 4 (Bidder’s Disclosure), SBD 6.1 (Preference Points Claim Form), Central Supplier Database (CSD) registration, SARS PIN, and updated CIPC company documents.

Skills Requirements

The tender does not mandate direct labour or technical service skills. Instead, it requires suppliers to demonstrate operational competence in medical device logistics, sterile product handling, and regulatory compliance management. Bidders must possess the administrative capacity to accurately complete mandatory returnable documents, manage timely deliveries, and coordinate sample submissions. Familiarity with South African public procurement frameworks and preferential scoring mechanisms is essential for compliant bid preparation.

• SBD Forms: Standardised government bidding documents used to verify bidder eligibility, disclose conflicts of interest, claim preference points, and confirm tax compliance status.
• Performance Security & Warranty: Successful bidders may be required to submit a bank guarantee or certified cheque within 30 days of contract award. The supplier warrants goods against design, material, or workmanship defects for 12 months after delivery or 18 months after shipment, whichever expires first.
• Five-Stage Sequential Evaluation: Quotations undergo strict, non-negotiable screening across five stages: administrative compliance, delivery capacity, specification adherence, price/preference scoring, and sample verification. Failure at any stage results in immediate disqualification without progression to subsequent evaluations.
