Objectives

This quotation seeks to appoint a supplier to provide and deliver AED (Automated External Defibrillator) pads that are compatible with the Mindray BeneHeart D3 defibrillator machine. The pads are intended for use at St Chads Community Health Centre in Ladysmith, KwaZulu-Natal, to support emergency heart rhythm analysis and defibrillation.

Scope

• Contract type: Once-off supply of goods.
• Quantity required: 10 sets, comprising 5 adult sets and 5 child/pediatric sets.

Technical Requirements

• The AED pads/electrodes must be compatible with the Mindray BeneHeart D3 defibrillator/monitor. The tender also refers to the model as “BeneHeart 03” or “DB3”, but the intended device is the Mindray BeneHeart D3.
• The pads must adhere to the patient’s bare chest and be capable of analysing heart rhythm and delivering a defibrillating shock where required.

• A SAPHRA certificate must be attached. This refers to the South African Health Products Regulatory Authority certificate required for regulated medical devices.

Skills and Compliance Requirements

• Bidders must be registered on the Central Supplier Database (CSD).
• Bidders must submit valid CIPC company registration documents.
• A B-BBEE certificate or sworn affidavit, for Exempt Micro Enterprises and Qualifying Small Enterprises, must be provided.
• Bidders must be tax compliant and may be required to provide a SARS PIN for verification.
• The quotation will be evaluated in stages: administrative compliance, capacity to deliver, compliance with specification, and price/preference point scoring.
• Failure to submit required administrative, compulsory, or mandatory documents may render the quotation non-responsive.

Preference Point System

• The 80/20 preference point system applies.
• Price carries 80 points.

• Proof such as CIPC, B-BBEE certificate, or sworn affidavit is required to claim the preference points.

Important Additional Terms

• Firm prices only are accepted; price variations will not be considered.

• The department is not obliged to accept the lowest or any quotation.
• Bidders must avoid conflicts of interest and complete the required disclosure forms if they or related persons are employed by the state or have relationships with Department of Health employees.
