The primary objective of this tender is to secure a compliant supplier for the one-off provision of medium-sized artificial eyes to support clinical operations at Ngwelezane Hospital, under the KwaZulu-Natal Department of Health.

Scope

The procurement covers the supply of 1,000 units of medium artificial eyes. The contract is a single transaction with no automatic renewal. Successful delivery must occur within three weeks (15 working days) of contract award. All supplied items must be brand new, unused, and retain a minimum shelf life of six months from the date of delivery. Pricing must remain firm for ninety days and include all transportation costs to the designated hospital location.

Technical Requirements

• Strict compliance with the attached product specification document is mandatory; failure to confirm compliance results in immediate disqualification.
• Suppliers must hold a valid license from the South African Health Products Regulatory Authority (SAHPRA) authorising the manufacture, wholesale, or distribution of medical devices.
• All goods must conform to applicable South African National Standards (SANS/SABS) and general quality benchmarks outlined in the bidding documents.
• A comprehensive warranty is required, covering twelve months post-delivery or eighteen months from shipment (whichever occurs first), guaranteeing freedom from defects in design, materials, or workmanship.
• Samples may be requested for evaluation purposes. Failure to provide samples when requested will result in rejection of the offer.
• Bidders must be fully registered and verified on the Central Suppliers Database (CSD) and maintain tax compliance with the South African Revenue Service (SARS).

• Anti-Cover Quoting Policy: Bidders attempting to submit competing quotes through multiple affiliated companies will be immediately disqualified for corruption and acquisition fraud.
• Mandatory Standard Bidding Documents (SBDs): Forms SBD 4 (Disclosure of Interest) and SBD 6.1 (Preference Points Claim) must be submitted exactly as printed. Redrafting, retyping, or incomplete submissions invalidate the quote.
• Performance Penalties & Termination: Late delivery incurs daily financial penalties calculated at the prime interest rate.
