Objectives

The primary objective of this tender is to procure books related to health records for adults, females, and children to support service delivery within the Department of Health at St Chads CHC. The focus is on acquiring standardized, quality health record books to facilitate effective patient documentation and health management.

Scope

The scope includes the supply of:

• 5000 units of health record books categorized as adult pink female and child records.
• Specific types of health record books for adult females and children, with detailed specifications on size, color, and material.
• Delivery to St Chads CHC within specified timeframes.

The procurement process covers administrative, technical, and quality compliance, ensuring the items meet specified standards and are delivered on time.

Technical Requirements

The technical specifications for the health record books are as follows:

• Color: Pink for adult female records; white for child records.
• Size: A4 (210 x 297 mm) with a booklet size of approximately 275 x 210 mm.
• Material: Bond paper (70gsm or 80gsm), with 84 pages per booklet.
• Packaging: Box of 150 units for child records; individual units for adult records.
• Design: Must include specified addendums and approved by the end-user.
• Samples: Not required for the adult records; samples for child records to be attached if specified.

Skills Requirements

The successful bidder must demonstrate:

• Experience in producing and supplying health record books or similar printed materials.
• Ability to meet quality standards and specifications as per the provided annexures and approved designs.
• Capacity to deliver within the stipulated timeframe (e.g., within 7 days of order receipt).
• Compliance with procurement policies, including proper documentation, disclosure, and adherence to legal and regulatory requirements.
• Provision of proof of previous similar work, including copies of orders and delivery notes.
