Objectives

The primary objective of this tender is to procure Female, Male, and Child Health Records to support patient documentation at Nseleni Community Health Centre. The records are intended for recording patient clinic information, ensuring proper documentation and management of health data for different patient groups.

Scope

The scope includes the supply of:

• 11,000 units of health records categorized as:
• Female Health Records (80 pages, printed on both sides, with specific layout and binding requirements)
• Male Health Records (80 pages, similar specifications to female records)
• Child Health Records (64 pages, designed for ages birth to 15 years, with specified content layout)

The records must meet detailed specifications regarding size, cover material, content layout, and binding, and are to be delivered to Nseleni CHC.

Technical Requirements

• Records must be printed on both sides with numbered pages and a glossy, pink-colored hard cover (300gsm) with the Department of Health logo.
• Specific content layout for each record type, including table of contents, demographic details, patient history, and other health information pages.
• Binding must be secure, not stapled.
• Records must conform to the detailed specifications provided, including size (275mm x 210mm), orientation (portrait), and cover design.
• Records should be of high quality, durable, and suitable for clinical use.

Skills Requirements

While the tender primarily focuses on the supply and quality of health records, the successful bidder should demonstrate:

• Experience in printing and binding medical or health documentation according to specified standards.
• Capability to produce high-quality, durable printed materials with precise layout and content accuracy.
• Ability to meet strict deadlines for delivery to the specified location.
