Objectives

The primary objective is to secure a one-off supply of high-quality duct tape for administrative use within GJ Crookes Hospital. The materials will be utilised specifically for colour-coding patient files to improve record organisation and retrieval efficiency.

Scope of Supply

• Product: High-quality duct tape rolls measuring 48mm in width and 25m in length.
• Quantities: 200 rolls per colour, totalling 1,200 rolls across six colours: yellow, black, red, white, green, and blue.
• Delivery: Single delivery to the hospital facility in Scottburgh. All transportation and handling costs must be included in the quoted price.
• Contract Duration: Once-off supply with a quotation validity period of 90 days.

Technical Requirements

• Products must be brand new, unused, and free from manufacturing defects.
• Expired or second-hand goods are strictly prohibited. All items must retain a minimum shelf life of six months upon delivery.
• Packaging must be commercially standard and robust enough to prevent damage or deterioration during transit.
• Goods must conform to applicable South African National Standards (SANS/SABS) where relevant.
• A warranty period of twelve months from acceptance or eighteen months from shipment applies, whichever occurs first.
• All submissions must utilise official, unaltered government quotation forms. Retyped or modified documents will be rejected.

Skills Requirements

No specialised technical, engineering, or installation skills are required. This is a standard consumable goods procurement. Suppliers must demonstrate basic logistical capacity to deliver the specified volume within the agreed timeframe, along with proficiency in maintaining public sector compliance, tax documentation, and accurate invoicing procedures.

Key Contractual & Procurement Terms

80/20 Preference Point System: Evaluation awards 80 points based on competitive pricing and 20 points for specific transformation goals. Full points are allocated to enterprises that are at least 100% owned by Black Africans, requiring valid B-BBEE verification or sworn affidavits.

Prime Interest Rate Penalty: Late delivery triggers daily financial deductions calculated using the prevailing prime interest rate until the goods are successfully delivered and accepted.

Cover-Quoting Prohibition: Submitting multiple quotations under different company names for the same tender is classified as fraudulent activity. Verification systems will cross-reference entities, and detected cases result in immediate disqualification.

SBD 4 Conflict of Interest Declaration: Bidders must formally disclose any personal, familial, or business relationships with employees of the KwaZulu-Natal Department of Health. Failure to declare constitutes a false statement and leads to bid rejection.
