Objectives

The primary objective of this tender is to procure customised identification labels for use in the Outpatient Department (OPD) at Prince Mshiyeni Memorial Hospital. These labels are designed to ensure accurate patient identification, prevent medication being issued to the wrong patient, and streamline patient waiting time management for priority cases.

Scope

The contract involves a once-off supply of specially formatted labels for the calendar year 2027. The scope encompasses:

• Supply of 251 rolls covering all weekdays (Monday to Friday), explicitly excluding weekends and public holidays.
• Delivery to the hospital's main stores at Prince Mshiyeni Memorial Hospital.
• A 90-day quotation validity period, with firm pricing and delivery timelines to be specified by the bidder.
• Adherence to standard government General and Special Conditions of Contract, including warranty, inspection, penalty, and dispute resolution provisions.

Technical Requirements

Bidders must strictly comply with the following specifications:

• Dimensions: 32 mm x 19 mm per label.
• Colour & Printing: All text, numbers, dates, and the "P" prefix (e.g., P1) must be printed in bold light green.
• Numbering & Layout: Each roll must contain two adjacent copies of every weekday date for 2027, with sequential numbering running from P1 to P500 per day.
• Packaging: Labels must be supplied on rolls.
• Quality & Condition: Products must be new, unused, and possess a minimum shelf-life validity of six months upon delivery.
• Sample Submission: A physical sample must be submitted on or before the closing date for technical evaluation against the specification.

Note on Unusual Requirements: The tender mandates a pre-closing physical sample submission for direct verification, which is uncommon for standard goods quotations. Additionally, the evaluation applies an 80/20 preferential procurement system, where 20 points are exclusively allocated to enterprises that are at least 51% owned by Black people, aligning with national transformation directives.
