Objectives

The primary objective of this tender is to procure 700 packets of Patient Referral Letters for Addington Hospital, KwaZulu-Natal, to support efficient patient referral processes within the healthcare system.

Scope

The scope includes the supply of:

• 700 packets of Patient Referral Letters, as specified in the item description.
• Provision of physical samples, full CSD report, and valid BBBEE certificate or affidavit.
• Delivery of the goods to Addington Hospital security tender box at Prince Street, Durban.
• Compliance with technical specifications, quality standards, and procurement policies.

Technical Requirements

The technical specifications for the Patient Referral Letters include:

• Sets of 4 leaves, gummed across the top, size 297mm x 210mm.
• Original (white NCR CB perforated), duplicate (white NCR CFB perforated), triplicate (blue NCR CFB perforated), and quadruplicate (yellow NCR CF firm).
• Printed in black ink, with specific perforation and packaging requirements.
• Packaged in boxes of 20 packets, each packet containing 50 sets of referral letters and 1 guideline form.
• Additional specifications include the use of 80 GSM white paper, lamination of fly leaves, and specific binding and labeling instructions.

Skills and Competency Requirements

Bidders must demonstrate:

• Ability to produce high-quality, NCR-compliant referral letter sets as per specifications.
• Provision of physical samples for evaluation and approval prior to bulk production.
• Compliance with quality standards, including SABS or SANS approval for the items supplied.
• Experience in manufacturing or supplying similar printed medical forms and stationery.
• Capacity to deliver the required quantity within the specified contract period.
• Provision of full CSD report and valid BBBEE certification or sworn affidavit.
