Objectives

The primary objective of this tender is to procure a range of physiotherapy mobility aids for St Chads Community Health Centre (CHC) in KwaZulu-Natal. Scope

• Walking Sticks (Pulpit Walker): 200 units
• Walking Frames (Adult): 10 units
• Buggy (Large) Adult: 4 units
• Buggy (Medium) Adult: Quantity not specified in the price page but listed as an item.

The contract is a once-off purchase, with a 90-day validity period required for the quoted prices. Delivery terms require the supplier to deliver the goods to the specified location, with all delivery costs included in the quoted price.

Technical Requirements

The tender specifies the following technical requirements for the goods:

• Crutches (Axilla): Must be made of wood, feature rubber handles and underarm pads, and have adjustable height.

• Mandatory Registration: Bidders must be registered on the Central Supplier Database (CSD). Failure to be registered or having a failed verification will result in disqualification.
• Tax Compliance: Bidders must be tax-compliant with the South African Revenue Service (SARS). A valid Tax Clearance Certificate or a SARS PIN for verification is required. Quotes from non-compliant suppliers will be disqualified.
• Pricing: Prices must be firm and remain valid for the 90-day period. The price must include VAT if the bidder is a VAT vendor. All delivery costs must be included. Non-firm prices will not be considered.
• Documentation: Bids must be submitted on the official forms provided and must not be retyped. All sections must be completed in full. Alterations must be initialled, and the use of correcting fluid is prohibited.
• Disclosure of Interest: Bidders must complete the SBD 4 form, disclosing any relationship with employees of the KZN Department of Health. Failure to disclose correctly will lead to disqualification.
• Preference Points: The 80/20 preference point system applies. Points are allocated as follows:
  • Price: 80 points
  • Specific Goal (RDP Goal): 20 points for promoting enterprises manufacturing or producing in KwaZulu-Natal.

Important Terms and Conditions

The tender includes standard government General Conditions of Contract (GCC) and Special Conditions of Contract (SCC). Key points include:

• Warranty: A 12-month warranty is required from the date of delivery and acceptance.
• Penalties: Penalties for late delivery will be calculated using the current prime interest rate per day of delay.
• Termination: The department may terminate the contract for default, poor performance, or fraudulent practices.
• Cover-quoting: The department will investigate and disqualify bidders suspected of cover-quoting (collusion).
• Negotiations: The department reserves the right to negotiate with shortlisted bidders if prices are deemed exorbitant or not market-related.
