1. Objectives

The objective of this tender is to appoint a suitable supplier to manufacture and supply staff uniforms for Housekeeper (Female) and General Orderly (Male) staff at Greys Hospital, KwaZulu-Natal Department of Health. The contract is a once-off procurement of goods.

The scope includes the supply of the following uniform items in specified quantities and sizes:

• Female Housekeeper skirts (fern green) - 63 units
• Female Housekeeper short sleeve shirts (fatigue) - 63 units
• Female Housekeeper fleece jackets (maroon) - 21 units
• Male long sleeve shirts (fatigue) - 18 units
• Male Housekeeper trousers (navy) - 18 units
• Male Housekeeper fleece jackets (navy) - 7 units
• Male auxiliary shirts (white) - 3 units
• Male Housekeeper trousers (black) - 18 units

3. Technical Requirements

All items must meet the detailed specifications set out in Annexure A, including:

• Fabric composition: e.g., 97% cotton/3% spandex, 100% cotton twill, 100% polyester fleece, 65% polyester/35% cotton
• Fabric weight: Ranging from 135gm² to 275gm² depending on the item
• Colours: Fern green, fatigue, maroon, navy, white and black
• Specific design features: e.g., double needle lapped seams, functional chest pockets, zip closures, adjustable cuffs, back vents, etc.
• Sizes: Various sizes from small to triple XL as specified per item
• Samples: Samples are required and will be requested from the top three scoring bidders
• Standards: Goods must conform to S.A.N.S./S.A.B.S. specifications
• Delivery: All delivery costs must be included in the quoted price

4. Skills and Compliance Requirements

Mandatory Administrative Requirements:

• Completed particulars of quotation and official price page
• Bidder's Disclosure (SBD 4) - declaration of interest
• Preference Points Claim Form (SBD 6.1)
• Completed specification form (Annexure A)
• Valid Central Supplier Database (CSD) registration
• Valid SARS tax clearance (tax compliance status verified on CSD)
• VAT registration number (if VAT vendor)

Preference Points (Specific Goals):

• 20 points allocated to companies at least 51% owned by Black People
• Proof required: BBBEE Status Level Verification Certificate or sworn affidavit (for EMEs and QSEs)

Evaluation Process (4 Stages):

1. Stage 1: Administrative compliance and mandatory requirements
2. Stage 2: Compliance with specification
3. Stage 3: Objective criteria - submission of samples (top three bidders)
4. Stage 4: Price and preference points (80/20 system)

Bidders must also disclose any relationship with KZN Department of Health employees, as failure to do so will result in disqualification. The department reserves the right to negotiate prices deemed exorbitant.
