Scope

This is a one-off quotation for the supply and delivery of eighteen (18) I-123 Sodium Iodide Capsules (5 mCi each) to Victoria Mxenge Hospital in KwaZulu-Natal. The contract validity period is ninety (90) days. The successful bidder will be responsible for end-to-end delivery, including all transportation, handling, and insurance costs, ensuring products arrive intact, within specification, and with adequate shelf life. The contract does not involve ongoing service delivery or maintenance obligations.

Technical Requirements

• Active ingredient must be Sodium iodide (NaI-123).
• Each capsule must deliver an activity of not less than 5 mCi upon delivery.
• Products must be storable at room temperature.
• Supplier must provide a valid SAHPRA compliance certificate for the product.
• All items must be brand new, unused, and possess a minimum remaining shelf life of six months from the date of delivery.
• Quoted prices must be firm for the 90-day validity period and include VAT (if applicable) and all delivery costs.
• Bidders must submit complete administrative documentation, including Central Suppliers Database (CSD) verification, tax clearance proof, completed Standard Bidding Document (SBD) forms, and the preference points claim form.
• Late submissions, expired products, or deviations from the stated specifications will result in immediate disqualification.
