Objectives

The primary objective of this tender is to supply and deliver patient health records, including female, male, and child records, in accordance with specified standards and specifications to Gamalakhe CHC. The aim is to ensure the provision of quality health record materials that meet the institution's requirements for effective patient record management.

Scope

The scope encompasses the manufacturing, supply, packaging, and transportation of patient health records as per the approved specifications, quantities, and delivery schedules. It includes the production of various types of health records (child, adult female, adult male), ensuring compliance with detailed specifications, quality standards, and delivery requirements to Gamalakhe CHC within the contract period.

Technical Requirements

• Records must conform to detailed specifications including size (216 x 300mm), cover quality (300g gloss printed full colour), inside page specifications (double-sided, no.1-64 or 84 pages), paper quality (80g bond white), and binding (PUR glue).
• All pages should be printed in black and white, with front cover logos and patient names in colour as specified.
• Records must include posted pockets on the inside back cover for easy insertion into pockets.
• Materials must be of good quality, free from defects, and suitable for routine use within healthcare facilities.
• Delivery must be made to the specified address with accompanying delivery notes.
• Records should meet the specified standards and specifications attached in the annexure.
