Objectives

The primary objective is to procure durable, specification-compliant patient record covers for clinical administration at Gamalakhe Community Health Centre. The procurement seeks to streamline patient file management while ensuring value for money, adherence to provincial supply chain governance, and alignment with national transformation policies.

Scope

The contract entails a once-off supply and delivery of fifty (50) boxes of patient record covers to the designated health centre. Each box contains ten packets, with one hundred covers per packet, resulting in a total quantity of fifty thousand units. The supplier bears all transportation and delivery costs to the prescribed destination. No installation, maintenance, or recurring services are included.

Technical Requirements

• Dimensions: 480mm × 365mm.
• Material: 225 gsm Kraft liner board.
• Printing: Single-sided application using black ink only, featuring a half-moon graphic centred on both the front and top cover.
• Construction: Pre-scored folding pattern (one score down the centre, two on each side) with side flaps folded and pasted onto the main cover area.
• Labelling: Front cover must clearly indicate contents and item numbers; all text must be in English.
• Packaging: Strictly configured as 100 covers per packet, 10 packets per box.
• Quality Assurance: Goods must match submitted samples and withstand normal handling conditions without deterioration.

Skills & Capacity Requirements

• Proven Delivery Experience: Validated through submission of at least one recent order confirmation and corresponding delivery note demonstrating prior supply of similar goods to public or private healthcare facilities.
• Administrative & Regulatory Compliance: Active Central Supplier Database (CSD) registration, valid CIPC business registration documents, and an up-to-date B-BBEE status certificate or sworn affidavit.
• Operational Capability: Demonstrated ability to manufacture, package, and deliver bulk orders within stipulated timeframes while maintaining consistent quality control.
• Documentation Proficiency: Competence in completing mandatory government procurement forms (SBD 4, SBD 6.1, GCC/SCC acknowledgements) without omissions or alterations.

Notable Terms & Conditions

80/20 Preference Point System: Evaluation uses a weighted scoring model where 80 points are allocated to price competitiveness and 20 points to socio-economic transformation. Full preference points are exclusively granted to enterprises that are at least 51% Black-owned. Bidders must submit verified proof (CIPC certificate, B-BBEE documentation, and CSD report) to claim these points.

Sample-Driven Award Process: Unlike conventional quotations, this process mandates physical sample verification. The three highest-scoring bidders will be contacted to submit samples; if all samples fail inspection, the entire procurement process is cancelled and restarted. This ensures tangible quality validation before contract award.

Delay Penalties & Termination Clauses: Late deliveries incur daily financial deductions calculated using the current prime interest rate applied to the delayed portion's value. Persistent non-performance, misrepresentation of documents, or engagement in corrupt practices may result in contract termination, financial claims for excess replacement costs, and potential restriction from public sector contracts for up to ten years.

Tax & CSD Verification: Suppliers must maintain valid tax clearance and accurate CSD profiles. Failure to validate tax status or provide required documentation renders the quotation non-responsive, regardless of pricing or technical merit.
