Summary: Supply and Delivery of Single Stage Straight Tip Venous Cannula

1. Objectives

The primary objective of this tender is to establish a contract for the supply and delivery of Single Stage Straight Tip Venous Cannulas to the Inkosi Albert Luthuli Central Hospital. The process is designed to ensure a transparent, fair, and cost-effective procurement that complies with the Preferential Procurement Policy Framework Act (PPPFA). The evaluation aims to select a supplier who can provide a compliant, high-quality medical device at a competitive price, while also promoting specific government socio-economic goals, particularly the empowerment of youth-owned enterprises.

2. Scope of Work

The scope involves the supply and delivery of specific venous cannulas used for cardiopulmonary bypass procedures. The contract is a once-off purchase for a total of 8 boxes, with each box containing 10 units. The required sizes are 12 Fr, 14 Fr, 34 Fr, and 36 Fr, with two boxes required for each size. The goods must be delivered to the hospital's specified location, and the quoted price must include all delivery costs.

3. Technical Requirements

The cannulas must meet strict technical specifications to ensure patient safety and procedural efficacy. They are used to decompress the heart during cardiopulmonary bypass. Key requirements are:

• Material & Design: Must be a venous drainage cannula that is transparent, wire-reinforced, and features a metal tip.
• Performance: Must not exceed a pressure drop of 100 mmHg to ensure adequate flow.
• Packaging: Supplied in boxes of 10 units.
• Compliance: Products must be new, unused, and have a minimum shelf life of six months upon delivery. Used or expired products will not be accepted.

4. Skills and Compliance Requirements

Bidders must meet specific administrative, legal, and financial criteria to be considered responsive. The evaluation process is staged, and failure at any stage results in disqualification.

• Mandatory Documentation: Bidders must submit a completed SBD4 (Bidder's Disclosure), SBD6.1 (Preference Points Claim Form), and the official price page. They must also provide a valid CIPC registration, a B-BBEE certificate or sworn affidavit, and a Central Supplier Database (CSD) compliance report.
• Regulatory Licensing: A valid license from the South African Health Products Regulatory Authority (SAHPRA) is required, authorizing the bidder to manufacture, import, export, or act as a wholesaler/distributor of medical devices.
• Disclosure of Interest: Bidders must disclose any relationship with employees of the KZN Department of Health, as per the SBD4 form. Failure to do so will lead to disqualification.
• Capacity to Deliver: Bidders must have a satisfactory delivery record. A history of failing to deliver on previous orders without acceptable reasons will result in being marked as a defaulter.

5. Evaluation and Award Criteria

The evaluation will be conducted in five stages. The first stage checks administrative and mandatory compliance. The second stage assesses the bidder's capacity to deliver. The third stage verifies technical compliance with the specification. The fourth stage evaluates price and preference points using the 80/20 system, where price accounts for 80 points and specific goals (youth-owned enterprises) account for 20 points. The final stage requires the top-scoring bidders to submit samples for quality verification before the contract is awarded.
