Objectives

The primary objective of this tender is to supply 750 units of Female Patient Health Records (PHC) to Umphumulo Hospital, KwaZulu-Natal, in accordance with specified standards and requirements. The delivery is a once-off requirement, with the goods to be supplied to Umphumulo Hospital, located at R 74 Greytown Road, Maphumulo, KwaZulu-Natal. Technical Requirements

• Supply of 750 adult female patient health record booklets.
• Booklet specifications:
  • Size: Approximately 275mm x 205mm
  • Cover: 230gsm material
  • Inner pages: 160gsm to 80gsm white board
  • Layout: Internal text printed with an internal pocket
  • Standard: Complies with SABS standards
• Product must be new, unused, and of current design models.
• Delivery to be completed as a once-off, with all documentation and packaging compliant with specified standards.
• All products must be supplied with relevant certification and documentation, including CIPC, BBB-EE sworn affidavits, and VAT registration if applicable.
