Scope

The contract covers a single, non-renewable supply of ten packs of Polyp Retriever Fork Nail Graspers. Deliveries must be made directly to the hospital premises, with all logistics and handling costs fully incorporated into the quoted price. Pricing must remain firm for a ninety-day validity period, and all delivered items must be brand-new, unused, and retain a minimum shelf life of six months from the delivery date.

Technical Requirements

• Instrument Specification: Polyp Retriever Fork Nail Grasper with eyelets, engineered specifically for grasping and extracting polyps during endoscopic procedures.
• Physical Attributes: 160 cm length, blue colour, plastic construction, supplied as individual units.
• Regulatory & Quality Compliance: Items must bear the SANAS logo and comply with applicable SANS/SABS standards. Products must be free from design, material, or workmanship defects and carry a standard twelve-month warranty post-delivery.
• Documentation: Bidders must provide a valid BBBEE certificate, a sworn affidavit/certificate, and proof of active Central Suppliers Database (CSD) registration. SARS tax compliance status must be verifiable.
• Submission Protocol: Quotations must be physically deposited in the designated tender box before the closing deadline. Electronic, faxed, or late submissions will be automatically disqualified.
