Objectives

The primary objective of this tender is to procure the supply of POPD forms and related printing forms (stationery) for RK Khan Hospital. The aim is to ensure the hospital has adequate printed materials to support its operational and clinical activities, complying with relevant standards and specifications.

Scope

The scope includes the supply and delivery of various printed forms such as follow-up visit forms, baseline visit forms, triage records, nursing registers, diabetes-specific forms, and ambulatory registers. The procurement covers both the physical forms and associated printing requirements, with a once-off contract period. The forms must meet specified standards, conform to SANAS/SABS specifications, and include necessary logos and certifications.

Technical Requirements

• Supply of various printed forms as detailed in the item list, including specific types like follow-up visit forms, baseline visit forms, triage records, and nursing registers.
• Forms must be printed on appropriate materials, conforming to SANAS/SABS standards.
• Must include the SANAS logo and be compliant with the specified specifications.
• All quotations must be submitted on official forms and not retyped.
• Quotations must be dropped off in the designated tender box at the main entrance gate of RK Khan Hospital.
• Pricing must include VAT where applicable, and the total quotation price should be valid for 90 days.
• Suppliers must provide valid BBBEE certificates, sworn affidavits, and SANS logo certification.

Skills Requirements

• Proven experience in printing and supply of medical forms and stationery.
• Ability to produce forms that meet SANAS/SABS standards and specifications.
• Knowledge of procurement procedures and compliance with government tender regulations.
• Capacity to deliver the forms within the specified contract period and in the required quantities.
• Understanding of the hospital’s operational needs and standards for printed medical forms.
• Competence in providing necessary documentation such as BBBEE certificates, sworn affidavits, and VAT registration details.
