Tender Summary: Supply of Sleeveless Reflect Jackets

Objectives

The primary objective of this tender is to procure 202 sleeveless reflective jackets for Nkonjeni Hospital, under the KwaZulu-Natal Department of Health. The quotation is a once-off purchase, and the jackets are intended for use by Community Health Workers (CHWs) as part of their identification and safety apparel.

Scope

• Goods Required: Supply of sleeveless reflective jackets (202 units).
• Delivery Location: Nkonjeni Hospital, Mahlabathini, KwaZulu-Natal.
• Contract Type: Once-off supply contract.
• Validity Period: Quotations must remain valid for 90 days.
• Procurement Process: This is a standard quotation process for amounts up to R1,000,000, subject to the Preferential Procurement Policy Framework Act (PPPFA) and its 2022 Regulations.

Technical Requirements

The jackets must meet the following detailed specification:

• Colour: Blue reflective vest.
• Material: Durable 100% polyester, breathable fabric.
• Features: Must include a zip and an ID pouch.
• Printing: Must have "Nkonjeni CHW" printed on the back.
• Sizes Required: The order includes a specific size breakdown: S (11), M (40), L (61), XL (26), 2XL (9), 3XL (6), 4XL (1), and an additional M (20) and L (28).
• Quality: Products must be new, unused, and have a minimum shelf life of six months. Used or second-hand items will not be accepted.

Skills and Compliance Requirements

Bidders must meet several administrative and compliance criteria to be considered responsive:

• Mandatory Documentation: Bidders must submit all required forms, including the official price page, Bidders Disclosure (SBD 4), and Preference Points Claim Form (SBD 6.1). Failure to do so will result in disqualification.
• Central Supplier Database (CSD): Bidders must be registered on the CSD, and their tax compliance status must be verifiable.
• Preference Points: The 80/20 preference point system applies. Points are allocated as follows:
  • Price: 80 points.
  • Specific Goals: 20 points. Full points are allocated to companies that are at least 51% owned by Black People. Proof, such as a B-BBEE certificate or sworn affidavit, is required to claim these points.
• Declaration of Interest: Bidders must disclose any relationship with employees of the KZN Department of Health. Failure to do so may lead to disqualification.
• Pricing: Prices must be firm and include all delivery costs. VAT must be included if the bidder is a VAT vendor.

Important Terms and Conditions

• Delivery: The quoted price must be all-inclusive for delivery to the specified destination.
• Penalties: Penalties for late delivery will be calculated using the current prime interest rate per day of delay.
• Termination: The Department may terminate the contract for default, poor performance, or fraudulent practices.
• Evaluation Process: Offers are evaluated in stages: administrative compliance, capacity to deliver, compliance with specification, and finally price and preference points. Only offers that meet the specification will be considered.
