Objectives

The primary objective of this tender is to procure services for the supply of patient health records, including specific types such as adult female, adult male, and child health records, in accordance with specified standards and specifications to support healthcare delivery at St Margarets CHC and other facilities within KwaZulu-Natal.

Scope

The scope encompasses the provision of:

• Design, printing, and supply of patient health record folders (adult female, adult male, and child health records).
• Adherence to detailed specifications regarding materials, size, print, packaging, and artwork provided by the client.
• Provision of samples upon request and compliance with quality standards.
• Delivery to specified locations within the KwaZulu-Natal province, with proper packaging and documentation.

Technical Requirements

The technical specifications for the health record folders include:

• Materials:
  • Ext: White Bond 70gsm (80gsm acceptable if 70gsm unavailable)
  • Cover: Sinavendar Board 230gsm
• Size: 275 x 210 mm
• Pocket: 165 x 215 mm
• Print:
  • Text web printed in black throughout
  • Cover litho printed in four process colours with overall matt varnish on one side, scored
  • Pocket plain, not printed, die-cut to shape and size
  • Folded, collated, perfect bound, with cover drawn on and trimmed flush
  • Pocket adhered to inside of back cover with Twinstix
• Packaging: 85 folders per box
• Artwork:
  • Client to supply a PDF file of the patient folder artwork for printing
