Objectives

The primary objective of this tender is to procure a supply of X-ray sets, including related forms such as request forms, to meet the needs of St Margarets CHC. The procurement aims to ensure the availability of quality X-ray sets that conform to specified standards and specifications, supporting effective diagnostic services within the health facility.

Scope

The scope encompasses the provision of:

• Supply of sets of X-ray request forms, including carbonized, perforated, and printed in English, packaged in specified quantities (e.g., 250 per packet, 10 packets per box).
• Delivery of the requested X-ray sets to St Margarets CHC at the specified location (Clydesdale Location, Umzimkhulu).
• Compliance with detailed specifications, quality standards, and packaging requirements as outlined in the tender documents.

Technical Requirements

The technical requirements include:

• Snap set of 3 leaves printed on one side only in black ink, with accurate registration.
• Size: 297mm x 210mm.
• Original white NCR CB-Perforated Triplicate and NCR CF-Perforated forms.
• Glued along the top with perforation 13mm from the top edge.
• Must be carbonized and comply with quality standards.
• Front cover labelled/printed to show contents and item number.
• Packaging: 250 leaves per packet, 10 packets per box.
• Samples must be available on request; failure to provide samples will disqualify the bid.
