The primary objective of this quotation is to procure a reliable and high-quality supply of specific wound dressings for Itshelejuba Hospital. The hospital aims to establish a contract with a capable supplier to ensure the consistent availability of these medical consumables for use in hospital wards and clinics, thereby supporting patient care and treatment.

Scope

The scope of this tender is the once-off supply and delivery of two specific types of wound dressings to Itshelejuba Hospital, located along the N2 Road between Piet Retief and Pongola. The contract is for a single delivery, and the quotation must remain valid for a period of 90 days. The required items are:

• Dressing Wound Acti-Coat Flex 3: A quantity of 10 boxes, each measuring 40cm x 40cm, of the brand Allewyn.
• Dressing Wound Therapy System: A quantity of 15 boxes of the PICO 7 system, each measuring 10cm x 30cm, of the brand Allewyn.

The successful bidder will be responsible for delivering the goods to the hospital, with all delivery costs included in the quoted price.

Technical Requirements

Bidders must supply products that strictly comply with the detailed specifications provided in the tender document. The key technical requirements are:

• Product Specification: The items must be exactly as described: "Dressing wound acti-coat flex 3" (40cm x 40cm) and "Dressing wound therapy system pico 7" (10cm x 30cm), both of the Allewyn brand. Bidders must confirm compliance with these specifications on the official form.
• Quality Standard: The products must be of high quality, new, and unused. They must not be expired and must have a minimum validity period of six months from the date of delivery.
• Packaging: The unit of measure is a box, and the supplier must ensure packaging is waterproof and sufficient to prevent damage during transit.
• Compliance: The goods must conform to the standards mentioned in the bidding documents and any applicable South African National Standards (SANS/SABS) specifications.

Skills and Administrative Requirements

Bidders must meet several administrative and compliance requirements to be considered. These include:

• Mandatory Documentation: Submission of all required forms, including the official price page, the Bidder's Disclosure (SBD 4), and the Preference Points Claim Form (SBD 6.1).
• Company Registration: Must be registered on the Central Supplier Database (CSD). Updated CIPC registration documents and a valid B-BBEE certificate or sworn affidavit (for EMEs/QSEs) are required.
• Tax Compliance: Must be tax-compliant with the South African Revenue Service (SARS), and a valid tax clearance certificate is required.
• Disclosure of Interest: Bidders must disclose any relationship with employees of the KZN Department of Health, as per the SBD 4 form. Failure to do so may lead to disqualification.
• Pricing: Quoted prices must be firm and include all delivery costs. If a bidder is a VAT vendor, the price must include VAT.

Evaluation Process

Quotations will be evaluated in four stages:

1. Stage 1: Administrative compliance and mandatory requirements (e.g., submission of all forms, CIPC, B-BBEE certificate).
2. Stage 2: Capacity to deliver, which checks if the bidder has a history of failing to deliver on previous orders.
3. Stage 3: Compliance with the technical specifications for the wound dressings.
4. Stage 4: Price and preference points. The 80/20 preference point system will be used, with 80 points allocated for price and 20 points for the specific goal of being a company at least 100% owned by Black Africans.

Bidders must note that late submissions will not be accepted, and the department reserves the right to not accept the lowest or any quotation.
