Objectives

The primary objective is to appoint a qualified and compliant service provider to manufacture, brand, supply, and deliver customized suggestion boxes to the municipality’s satellite offices, enabling structured internal and stakeholder feedback collection.

Scope

The contract encompasses the complete provision of thirty (30) suggestion boxes. The appointed provider will be responsible for material procurement, fabrication, official branding, quality assurance, packaging, and final delivery to multiple designated satellite office locations within the district.

• Branding & Labeling: Each box must feature the official Sekhukhune District Municipality (SDM) logo and prominently display the text “SUGGESTION BOX”

• Delivery: Coordinated drop-off at various satellite office addresses as specified by the municipality

MBD 4 declares potential conflicts of interest; MBD 8 verifies tax compliance; MBD 9 confirms adherence to preferential procurement policies; MBD 6.1 pertains to industry-specific registration where applicable. All forms must be fully completed and manually signed.
• CSD Summary Report: An official verification document issued by the National Treasury’s Central Supplier Database, confirming the bidder’s active registration, banking details, and compliance standing.
• 80/20 Preferential Scoring System: Bids are evaluated using a dual-criteria model. Eighty percent of the total score is awarded based on competitive pricing. The remaining twenty percent is distributed through a points-based preference system targeting historically disadvantaged groups, specifically: black South Africans (12 points), women-owned enterprises with over 50% ownership (2 points), disability-owned enterprises with over 50% ownership (2 points), youth-owned entities (2 points), and locally registered businesses (2 points).
• Returnable Documents: Mandatory attachments that trigger immediate disqualification if omitted or non-compliant. These include valid company registration certificates, certified director identification documents (not older than three months), proof of municipal rates payment (or a sworn affidavit for rate-exempt rural registrations), and full tax clearance documentation.
