Objectives

The primary objectives of the tender are to establish a comprehensive incident management framework aligned with Eskom’s SHEQ policy, promote proactive reporting of near-misses, ensure prompt and appropriate investigations, facilitate learning from incidents, share incident information, and implement corrective actions to prevent recurrence. The process emphasizes a culture of safety, zero harm, and continuous improvement in occupational health and safety performance.

Scope

The scope encompasses the management of occupational health and safety incidents across all Eskom Holdings SOC Ltd entities, including subsidiaries and controlled entities. It applies to Eskom employees, contractors (unless explicitly excluded), and members of the public affected by Eskom activities. The procedure covers all incident types resulting in injury, illness, property damage, or fatalities, including near-misses, occupational diseases, and environmental hazards, both within and outside workplace boundaries where relevant.

The procedure is applicable to incidents occurring during work activities, on-site, during commuting (if related to work), or in the course of employment, including those involving contractors and third parties. It also addresses incidents involving criminal activities, occupational diseases, and environmental hazards, with specific provisions for investigations, reporting, and classification.

Technical Requirements

• Implementation of incident identification, reporting, investigation, and classification processes.
• Use of standardized investigation templates and root cause analysis techniques.
• Timely reporting of incidents to relevant stakeholders, including legal and regulatory bodies.
• Preservation and collection of evidence at incident scenes, ensuring integrity and chain of custody.
• Classification of incidents based on severity, consequence, likelihood, and risk assessment matrices.
• Management of corrective and preventive actions with defined start and end dates, verification, and closure.
• Regular monitoring, review, and reporting of incident trends and corrective actions.
• Adherence to legislative requirements, standards, and Eskom policies related to occupational health and safety.

Skills Requirements

• Knowledge of occupational health and safety legislation, standards (ISO 45001, ISO 9001, OHSAS 18001), and Eskom policies.
• Proficiency in incident investigation techniques, root cause analysis, and risk assessment.
• Ability to collect, preserve, and analyze evidence in incident scenes.
• Strong communication skills for incident reporting, investigation findings, and stakeholder engagement.
• Competence in using SAP EH&S or equivalent incident management systems.
• Understanding of legal and regulatory reporting requirements.
• Capability to develop and implement corrective and preventive actions.
• Experience in managing multidisciplinary investigation teams and facilitating independent investigations where necessary.
